COVID-19 VACCINE NOTICE
If you are a patient in this practice, you can now book your own appointment. Please click on “Schedule Appointment” above, select “COVID Vaccine”, and choose an appointment time. The second dose will automatically be scheduled 3 weeks later at the same time on that day.
NOTE: This is open to the public for adults ages 18 and over, and individuals 5 up to and including 17 years old with parent/guardian permission. If you are not a patient, please call our office to schedule an appointment.
*If you are under 18 years old and coming to your vaccine appointment alone, please download our COVID-19 Vaccination Consent Form and have a parent/guardian sign it. You will need to bring this to your vaccine appointment.
After you have successfully scheduled your appointment, you will need to fill out the New York State COVID-19 Vaccine Form. You will need to complete this step in order to receive the vaccine. Please save the confirmation of completing this form to show to your vaccine administrator.

Reporting At Home Positive Test Results
If you or your child use an at home test and receive a positive result, please take a picture of the result. The picture can be uploaded to the health department and/or school. Please go to monroecounty.gov/health-covid19-reporting to report your result.

We have 2 suppliers for our flu vaccines:
One for Employer-based insurance plans
and another for State insurance plans. State insurance plans include:

- MVP Option
- Child Health Plus
- United Healthcare Medicaid
We have an Open House
scheduled on Wednesday, January 19th, at 5:30 pm

This is a great opportunity to find out more about our practice. Please call our office to register for the event. This will be an in-person event
Our practice uses a triage system for calls when our office is closed. Nurses, with specialized training in providing pediatric medical advice, answer calls to our office phone line, (585) 225-2525. One of our pediatricians is available 24 hours a day 7 days a week as backup for the triage nurse and will return calls when appropriate.
*We are participating members of AHP.
If your child has not had his/her annual physical, please call to make an appointment before the end of the year.
All patients will receive a completed Health Appraisal form at the annual physical. Additional requests, as well as camp and sport forms, will be subject to a $5 processing fee.
As a part of your medical home, you can expect:
- Same-day appointments
- Acute care for illnesses & injuries
- Preventative care, well-child visits & immunizations
- Behavioral health screening
- Patients can be seen for urgent & routine appointments during extended hours.
