Office Visits
For the convenience of our patients, we do schedule visits by appointment. When you are making your appointment, please let us know of any changes to your insurance, address or phone number. In addition, please call us if you know that you will be delayed or cannot keep your appointment.
OUR PROVIDERS FOLLOW THE WELL VISIT SCHEDULE SET BY THE AMERICAN ACADEMY OF PEDIATRICS:
Acute Visits – Short visits with a provider to address minor issues.
Office Visits – Intended for medication management by the patient’s primary provider.
Consultations – Extended visits with the primary provider for an in-depth review of a particular problem.
Nurse Visits – Visits that do not require a physician or a nurse practitioner.
In addition to office visits, keep in mind that all of our providers are on staff at all four area hospitals, including Golisano Children’s Hospital at Strong, Highland Hospital, Rochester General Hospital and Unity Hospital.
FORM REQUIREMENTS
We require five business days to complete any forms. If you would like your form mailed back to you, please include a self-addressed, stamped envelope.
School, sports, camp and daycare forms require a recent exam, as well as a signed Consent to Release Medical Information Form to have your child’s medical information released.
In order to fill pre-operative forms, we require an exam within 30 days of the surgery.
